Sickness Certificates

If you take time off work you may self certify for up to 7 days. Thereafter, you will need to contact your GP to obtain a sickness certificate (Form Med 3). If you require time off work as a result of a hospital admission or out patients appointment the hospital should provide you with a sickness certificate for the duration of your anticipated incapacity or recommended period of recuperation. Ask at the hospital before being discharged.
